Our services are critical components of primary care that promote better health and reduce overall health care costs for the patient. The goal of this program is to improve patient's health and well-being by reducing care gaps, developing lifestyle goals, connecting patients to resources, and facilitating access to doctors to help manage their chronic conditions
The Care Navigator provides ongoing, proactive patient services through regular communication and personalized care plans. They assess patient health status, address concerns, and facilitate access to community resources to improve overall well-being.
